Training is provided through the State of Wisconsin with $500 in cash bonuses paid upon completion of the certification. Responsibilities and Job Duties

• Provide personal assistance support direction care and supervision to up to (4) clients in a smaller home-like environment.
• Perform household tasks; cleaning basic home related chores and assigned client care tasks throughout each shift.
• Perform charting and documentation medication assistance and monitoring for symptoms.
• Prepare and serve (3) meals daily according to company menu. Prepare and submit food and supply order lists weekly.
• Assist and supervise clients in learning Independent Living Skills.
• Assist and direct clients in Activities of Daily Living.
• Plan and participate in activities plan weekly outings and crafts with clients in the home.
• Work with the supervisory team to ensure compliance safety and cleanliness in the home.
• Work as a team with other direct care staff and supervisors.

POSITION REQUIREMENT:
